IMPORTANT REGISTRATIONINFORMATION YOU NEED TO KNOW

Fax Registration forms:
• Fill out Registration Forms completely.
• Include Job Number or P.O. Number if applicable.
• All Registration Forms must have a Contact Persons name.
• Limit one day to each Registration Form. (Example: If student is taking classes for more than
one day, use different forms for each day.)
• All Faxed Registration Forms are needed by 12 noon on the day before training is to be
requested. Any student that arrives for class, without being registered, will have to wait until
all registered students are checked in. The student will be asked to contact his company to
have a registration form faxed. Once the fax is received, the student will be placed in the next
available class.
• The council CAN NOT alter a registration form. If a student is scheduled for a class and can
show proof that they have a current card; the company will have to resubmit the registration
form indicating that the student is not required to take the course.
• The Student will only be placed in those classes that they are scheduled for. The Council will not make the decision as to which classes that we think that the student needs.
• Plant site-specific courses can only be taken if proof of a current Basic Plus™ or Basic Plus
Refresher™ can be verified. Plant area-specific courses can only be taken if proof of a
current subsequent site-specific course can be verified.
• If changes need to be made to a registration form, make the changes and write, “REVISED”,
and then resubmit.
• PLEASE PRINT the student’s name on the registration form one time. List all of the training
for that individual by the student’s name.
• For specialty classes, please call the Safety Council to schedule a day and time for the special
class requested.
• DO NOT over schedule the student for class. Allow the appropriate time to finish classes. If
time does not allow a student to finish all required courses, use another registration form to
schedule the student for remaining classes for another day.
• By registering a participant it is understood that there will be a charge for no-shows, and Failures. NO SHOWS- There will be a $10 office fee for no-shows. If you have an employee scheduled for a class and he/she cannot make it, PSTC must be notified by 5 pm the first business day prior to the scheduled class date. FAILURES- You will be charged the regular class fee for your employee, weather they pass or fail.
Replacement Badges:
There is a $10.00 charge for any replacement badge. This amount must be paid in cash. If the
Contracting Company agrees to pay this amount, a Registration Form must be completed and
faxed to us indicating student’s name and social security number. Indicate “Replacement
Badge (ID)” in the upper right corner on the form.
Proper Identification:
Basic Plus/Basic Refresher and Confined Space Courses:
Attendees must present a valid form of photo identification. Valid is defined as legal and
legitimate. Identification must be original documents; no copies or facsimiles are accepted.
The identification must ALSO meet the following criteria:
• A valid state or federal government issued identification card or license that contains a
current photograph. This document must be un-expired or expired no longer than 60 days
with a valid receipt of renewal.
• A valid passport that contains a current photograph.
All Other Training Courses:
• Subsequent to any trainee being admitted for training, he or she must present a valid
picture I.D.
• Prior to receiving a badge from the badge-issuing window, an I.D. must be presented.
• Persons taking Site Specific Training, must show proof they have a current Basic Plus
prior to site training.
We believe that this policy will help all parties involved assure that the training history
of an individual is accurate.
Reciprocity Badges for the Basic Plus Orientation:
Any student that has successfully completed the Basic Plus ™ Orientation from any of
the Reciprocal Councils must be able to show his/her safety badge from that council. The
badge must be current to be effective. This will allow us to transfer the Reciprocal
Basic Plus™ to their PSTC badge. The student will not have to retake the Basic
Plus™ Orientation at our location.
Computer-Based Training:
Many of the PSTC’s courses are computer-based. The CBT (Computer-Based
Training) lab is open Monday through Friday 7:00 am until 2:00 pm. These classes have no
specific start time, although the same registration procedures for instructor led classes are required for CBT classes. CBT courses are self-paced: therefore, class time will vary. No trainee will be allowed to start a CBT course after 2:00 pm Monday through Friday. |
